New Patient Registration
If you are new to the area and live within our catchment area, you are welcome to register with us as a patient.
If you would like to register with us, you can do so online by using the following link:
Register with a GP Surgery (gp-registration.nhs.uk)
or by coming to the practice to complete a registration form (GMS1) and health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
Please bring with you a proof of your current address, (dated within the last three months) and photographic identification.
If you have a medical card or know your NHS number, please bring this with you as it will help us to speed up the process and obtain your medical records.
Once you have completed your registration form (GMS1) and the health questionnaire, please hand the documents back to our team at the frontdesk.
Once you have handed in your completed documents, theregistration process can take up to 10 working days to process. Medicaltreatment is available from the date of full registration.
You will be registered with the surgery and not a specificdoctor (despite one of their names appearing on your new medical card when youreceive it). Any doctor will be pleased to see you, or you may prefer torequest a specific doctor; please ask reception. We will endeavour toaccommodate your preferences; however, we wish to remind you that this may notalways be possible.

Somenew patients will be recalled for a New Patient Check with our nursing team.You will need to bring a urine specimen to this appointment. This health checkwill help us to become acquainted with you and your health needs.

Congratulations on your new arrival!
We’d like to remind you of the importance of booking and attending two key appointments:
The 6–8 Week Postnatal Check: A vital health check for both you and your baby, to ensure you are recovering well after birth and that your baby is developing as expected.
The 8 Week Vaccinations: The first in a series of routine immunisations to help protect your baby from serious illnesses.
Please note: Your baby must be registered at the practice before these appointments can be booked. If you haven’t already done so, please contact us as soon as possible to complete the registration.
Attending these appointments on time is one of the best ways to support your and your baby’s health.
